Nursing

(RN/LPN)

Nursing service will provide assessment of newly referred patients and provide skilled nursing on assigned patient according to medical orders.

Home Health Aide

(HHA))

Following are some of the services provided by the HHA:

  • Personal Care
  • Diet & Meal Preparation
  • Dressing
  • Grooming
  • Medication Reminders
  • Assist with ADL's (Activities of Daily Living)

Personal Care Aide

(PCA)

The PCA will provide the following services:

  • Bathing
  • Dressing
  • Grooming
  • Walking

Homemaker

The Homemaker service includes:

  • Assures Patients Safety
  • Assists with Personal Laundry
  • Helps with Meal Preparation
  • Escorting Patient to Appointments

Housekeeper/ Companion

The Housekeeper/Companion services includes:

  • Laundry
  • Linen Change
  • Bed Making
  • Dusting
  • Dish Washing
  • Running Errands

CDPAP/CDPAS

A New Programapproved by NYS Department of Health

The Consumer Directed Personal Assistant program (CDPAP) also referred to as the Consumer Directed Personal Assistance Services (CDPAS) is an alternative to the traditional Home Care Services. It is a self-Directed home care program available to Medicaid Eligible Consumers who are in need of home care services and are capable of directing their own care. Consumers must be eligible for New York State Medicaid. Your Medicaid Managed Care Provider will conduct an assessment to determine the amount of hours you are eligible under the CDPAP program.

Once your eligibility is determined, the next step will be to find a Personal Assistant (PA)
  • The PA(s) can almost be anyone the consumer wants.
  • The PA could be a neighbor, friend, aide or even a family member. The PA does not have to be HHA certified.
  • A parent, spouse, or designated representative cannot be a PA.
  • Your PA will need the following in order to start work:
    • - A physical within the past year
    • - Proof of immunization
    • - A PPD
    • - Health Assessment
    • - Proof of eligibility to work in the United States

Respite Care

The focus of Respite Care is to provide a short term period of relief or rest for the full time caregiver, from the continuous caring of a loved one. Some of the services provided are:

  • Assistance with personal care i.e. dressing, grooming
  • Accompanying client to a doctor’s appointment
  • Grocery Shopping
  • Running errands e. g. picking up prescriptions
  • Meal Preparation
  • Light housekeeping
  • Laundry and linen changes

Hospice Care

Hospice Care is generally provided by a team of health care professionals with their main focus on providing constant care, privacy and comfort to the terminally ill client, whether that is, by addressing the client’s physical comfort, spiritual needs, pain management, counseling, or general support and overall well being of the client.
